We look forward to having your family at St. Paul's Preschool this year! Please print and complete the enrollment forms below and bring them to the office to complete your registration & enrollment process.  

• All families must:
  • Complete an enrollment packet, including Physician’s report and Immunizations.
    • The physical exam form must be returned within 30 days of enrollment, otherwise, the child will be excluded from the program. 
    • Immunizations- A child needs to be in compliance with California State Immunization Laws/Regulations. For a list of required immunizations refer to the following website: http://www.shotsforschool.org/child-care/
  • All necessary documents must be on file prior to attending school.  Final registration and tuition are handled by the front office.
  • Inform the front office in a timely manner of any changes in address, phone number, family, and emergency information.
